The first area is especially insane because of crowd of enemies in the small room. You can't avoid their attacks at all, so you have to maintain health for many soul sphere. After falling the first area, you might be surprised by a herd of frozen cacodemons but they are not activated yet until you go around this map and return to the area. I dealt with the cyberdemons after getting the BFG, then a hell knight was in a good position for infighting in this run, so I was able to do it safely.
