Accidents
0:06~0:08 : STS-51-L. Space Shuttle Challenger exploded during launch, killing all 7 astronauts on board. Launched 28 January 1986, exploded after 73 seconds into launch.

0:09~0:10 : STS-107. Space Shuttle Columbia was rapidly disassembled over Texas during re-entry, killing all 7 astronauts on board. Launched 16 January 2003, got disassembled during re-entry over Texas, 1 February 2003.

0:11~0:12 : Space Shuttle Columbia being transported to Kennedy Space Center for the STS-1 mission, however, during the transport process on top of a modified Boeing 747, over 100 thermal plates were detached from the orbiter and fell into the ground. The first launch of the space shuttle, STS-1, was supposed to be in 1978, but due to the reasons above, the launch of STS-1 was delayed to 1981.

0:18~0:21 : Apollo 13. "Houston, we had a problem". No need to explain, I bet that all of you guys know what happened to Apollo 13. Launched 11 April 1970, returned 17 April 1970.

0:21~0:22 : STS-27. A secret military mission. A material fallen out from the right solid rocket booster(SRB) hit Space Shuttle Atlantis' heat-protection system. Damaging a few thermal plates. Launched 2 December 1988, returned 6 December 1988.

0:22~0:24 : AMOS-6 mission. SpaceX's Falcon 9 rocket exploded on launchpad SLC-40 during a static fire test, destroying the rocket and the SLC-40 launchpad itself. Exploded 1 September 2016. SLC-40 is currently restored, and it is up in running.

0:12~0:15 : Space Shuttle. Started in 1981 with the STS-1 mission, and ended in 2011 with the STS-135 mission.

0:15~0:16 : Constellation Project. It was supposed to be a human moon landing mission, but it was canceled due to NASA pushing back the tests of the rocket. This was caused by the STS-3xx mission, a mission to rescue astronauts who are stuck in orbit due to a problem/problems on the space shuttle they are on. Never launched.
