The six variations of wild mushrooms and one specials mushroom (monarch mushroom), when auto-sorted, from left to right in the inventory are:
1. Cured Heal Type 1 ( Restores 50% HP & 7,5% RP, Can clear some status ailments such as cold, sick, fatigue, & dizzy. )
2. Cured Heal Type 2 ( Restores 100 HP & 50 RP, Can clear some status ailments such as poisonus, paralysis, & seal )
3. Mushroom will gives sleep status ailment, ( Restores 10% of HP & RP, cause sleep after eating it and will temporarly disables for a short duration. )
4. Mushroom will gives cold status ailment ( Restores 10% of HP & RP, will inflict cold/sick status ailment. RP is reduced every few seconds based on your MAX RP. )
5. Mushroom will gives paralyze status ailment ( Restores 100 HP & 50 RP, will inflict paralyze status ailment. cause prevents character for running. )
6. Mushroom will gives poisonus status ailment ( Restores 100 HP & 50 RP, cause poisonus effect after eating it and receives poisonus damage every few second based on your MAX HP. )

1. Selphia Plain Areas (South / South West Areas) 100%
2. Autumn Areas (Near of Pond Autumn / Green Mushroom) 75%
3. Sechs Territory (East of House) 50%
Details :
Idra Cave = left map, those 5 stumps.
Idra Cave = go right 2 map, those 2 top mushrooms.
Sechs Territory = The map you teleported to have a stump with mushroom.
Sechs Territory = Up 2 map - Right secret short cut, this map have 2 stumps with mushrooms.
Sechs Territory = Up 2 map - Right secret short cut - up - right 2 map - the mushroom on the top right.
Sechs Territory = Up 2 map - Right secret short cut - up - right 2 map - up - the stumps here.
Maya Road Entrance - right 1 map.
(Detour) Maya Road Entrance = right 3 map (rune crystal here) - up (rune crystals here.)
Maya Road Entrance = right 3 map - up 2 map - middle top mushroom and top right mushroom.
Maya Road Entrance = right 3 map - up 3 map - go around the pond, the mushroom at the top.
Maya Road Entrance = right 3 map - up 2 map - right 1 map - except the bottom mushroom and top most left mushroom at the background, the other 5 mushrooms can spawn.
Revival Cave = right 1 map - those 2 mushrooms at the top.
Revival Cave = right 2 map - those 2 stumps.
Revival Cave = use teleport skill 1 time, now you are back at autumn road, there's 1 mushroom here.
Autumn Road = go down 1 map - go left - the mini stump at the top, and the giant stump in the middle.
Autumn Road = go down 1 map - go left - go down - the 2 stumps here.
Autumn Road = go down 2 map - I know the left most and right most can spawn, I don't really know the middle ones since I got Monarch mushroom before I can verify.
Keeno Lake = Mushroom here
Keeno Lake(take mushroom from every map) = up - right - down - right - up (there's mushroom here on the right side of the rock, behind the giant mushroom.) - right.
