My Launcher settings: -novid -tickrate 128 -high -threads 4 +fps_max 0 +cl_interp 0 +cl_interp_ratio 1 +rate 128000
+cl_updaterate 128 +cl_cmdrate 128 +mat_queue_mode 2 -freq 60 -refresh 60 +cl_forcepreload 1 -nod3d9ex -nojoy -console

Note: set number of threads acording with your Cpu, freq & refresh acording with the monitor also remove -console if it's to annoying for you.

And yes +fps_max 0 unlocks the limit that it's set default by valve!
