After several hours of farming +1 weapons on hard mode local, I swapped to normal local to get philosopher stones. You won't believe what happens next.

Happened randomly, not sure how to replicate. Surprisingly very hard to platform while the game's graphics are garbled. I hope my PS3 is not melting.
Recorded on real hardware, turning the PS3 system off fixed it.
