You have six buttons, no joystick, each one corresponding to one of the six areas of the screen where enemies can appear, the areas being laid out in the two-rows-of-three grid.
Every button shoots bullets on the screen and it can be pressed multiple times, you will be facing different enemies from different stages, sometimes one hit is not enough.

Some enemies will shoot you and your life bar (top left) will slowly empty, once depleted the game is over.and you can choose to continue or give up.
You also need to pay attention to the hostages, they pop up randomly on the screen, you lose health if you hit them.
But to be honest is every hard to avoid them, so shoot more terrorist/monsters then them. Be sure to miss less and kill more or your rank will be bad.

After you empty the enemies meter up-right, the stage ends and you're shown some statistics with a rank.
Like said before there are 6 stages in the game with increasing difficulty, the last two stages are "crazy" hard.

Once you complete all the stages, you'll receive a kiss a prominent young lady and that's it.
