Use either the automatic or manual installation method

If using the manual method, download the file called SUS.7z, make sure to extract all file contents into your Dark Souls 3 'Game' folder. File directory route for Game folder will look like this,

SteamLibrary\steamapps\common\DARK SOULS III\Game

Now just open up the DS3T.exe file and select the Graphics tab. In here using the drop down box select aspect ratio correction to ON. Then make any other changes you want and close the program and off you can go and play!
