So first of all, Permadeath is a concept in which when either you, or something you're in charge of, must stay alive as long as possible. What happens when you lose all your hit points? You lose that character forever.
In Demon's Souls, when you lose your life in game you also drop your souls and start back at the last bonfire.

This is called a Permadeath-Ish

What the -Ish in Permadeath-Ish means is that when I lose my life, I have to retrieve my souls or else if I lose my life again before that, then that's it, game over.


So TL;DR:
I have to retrieve my souls before I die again in order to keep playing.
