This is Hell Revealed 2 Level 15: The Path II - done on Ultra-Violence with all kills and secrets, played through PrBoom + v2.5.1.4.
The Path II is the fifteenth level of Hell Revealed 2 and it was designed by Jonas Feragen. The original Hell Revealed had a level titled "The Path", which played out a bit like this map.

The Path 2 has 783 monsters, which is by far the highest of any HR2 maps prior to it. The first two big rooms are going to determine how you'll do the rest of the map. For me, things went quite smoothly, considering that I just opened PrBoom and started playing the map. Unfortunately, some mistakes were made along the way, some of which could have easily set me back to the starting room plus I could not get 200% health for MAP31.
