Final map. The final area has a few new enemies. You will notice that it can fill up quickly with green spirits and there is a reason for that.
The pain elemental has been changed and shoots out a green projectile that can hurt you. If the projectile hits a wall then a new type of lost soul will spawn.
The green spirit, which has replaced the lost souls, can shoot projectiles at you that are similar to a baron projectile. Can be very dangerous. They can be taken down with 1 shot from the pistol due to having little health.
The green imp, which replaces the monster spawner, also shoots the same green projectiles as the pain elemental so it will spawn the green spirits if it hits the wall.

Now, this is just speculation but it looks like there might have been more new monsters as well. There are some changes to monsters that aren't seen in the wad. It's hard to tell what some of these might be since they were never complete.
The Arachnotrons' plasma rounds have changed to the green projectiles that spawn the green spirits. It appears to have been switched to one of the black pole Thing. In some frames, it will stay as the arachnotron and shoot out the green blobs, but in others, it will turn into the black pole and shoot rockets.
The mastermind also has been switched to the black pole Thing and only shoots rockets. It can also fly around. All the frames were changed to said object.
The SS soldiers have new sounds, more health, and fires multiple mancubus projectiles quite quickly.
Commander Keen acts like a green projectile turret. It continues to shoot out the projectiles as long as you're in its sight, creating lots of green spirits.
Icon of Sin/Romero has no sounds or pain frames.
