This is Doom 64: Retribution Level 21: Pitfalls - done with all kills and secrets on Watch Me Die skill.

Pitfalls is the twenty-first level of Doom 64. Pitfalls is a bit maze-like and it involves a lot of backtracking, which I'm not a fan of. Also, some switches do not give away their functions, which is another thing adding the confusion factor.

Pitfalls is a bit strange level in terms of its theme, it doesn't really look anything you have come across before, but it is a good thing in this case. Also, that weird music is good way of adding extra disturbance when you wander around the maze part.
