I did some testing to figure out what exactly causes the silver key in 3-V (lizard town) to despawn. This involved isolating every possible action that I could think of, and checking the silver key house after each action. This was pretty tedious and I don't recommend actually watching this footage, but here are my results. The following two actions can cause the 3-V silver key to disappear, and do so consistently:

-- Grabbing the 3-V silver key (no grabbing it twice after all!)
-- Grabbing the 4-2 silver key (the icy level)

The following actions did NOT cause the 3-V silver key to disappear:

-- Grabbing a red heart
-- Using a red heart
-- Grabbing a yellow heart
-- Using a yellow heart
-- Grabbing a revival capsule
-- Using a revival capsule
-- Opening a silver chest
-- Opening a gold chest
-- Entering the 3-V silver key house with any particular character
-- Dying as a character
-- Beating any level after 3-V
-- Equipping the translation guide in 3-V
-- Equipping the briefcase in 3-V
-- Talking to the lizard king to save the other villages
-- Acquiring the gold gem form the lizard king
-- Acquiring Dorami
-- Grabbing any of the gold keys in the game
-- Grabbing any of the silver keys in the game EXCEPT FOR the 4-2 silver key
-- Saving the forest in 1-1
-- Illuminating 1-2
-- Illuminating 4-1
-- Purifying the water in 3-1
-- Removing the fog in 3-2
-- Flipping the switch in 2-3
-- Grabbing the two crystals in 2-Boss
-- Talking to the girl in 3-T that allows you to get the gold gem from the lizard king (yes, I actually tested this lol)
-- Obtaining the floppy disc
-- Giving the floppy disc to the man in 3-T
-- Giving the gold gem to the man in 3-T
-- Watching the cutscene before 4-B
-- Beating Gundam 2 in 4-B and then exiting the level
-- Using the party popper in 4-2 and not grabbing the silver key
-- Using the fan in 4-2 and not grabbing the silver key
-- Exiting the 3-V silver key house without grabbing the silver key
-- Using the infinite breath item in 1-4
-- Using the infinite breath item in 3-3
-- Purifying all items

There are obviously some things I did not check (using the party popper/fan in 4-2 and exiting without beating the level, watching the intro cutscene for world 4, checking the 3-V silver key house BEFORE completing the levels before 3-V, etc.), but this is a pretty solid list of actions that have now been tested and recorded.

Also worth noting is that grabbing the 3-V silver key does cause the 4-2 silver key to despawn. This means it is (currently) impossible to collect all 17 known silver keys in the game. I have NOT tested if grabbing the 3-V silver key is the only action that causes the 4-2 silver key to despawn. I bet that's the case, but it has not been tested in the same way as the 3-V silver key despawn.
