No approval meter, When using a controller on PC the mouse pointer remains on screen (Changing to windowed then switching to fullscreen seems to fix it), Companions animations keep reseting while chatting with them, Sound effects randomly cutting out, Being unable to switch characters after dismounting your mount, Armor not rendering properly on Qunari inquisitors, Qunari arms glitch out when using Bow related skills or closing rifts,

Sometimes voice commands in multiplayer stop working, Quest items being marked as valuables and thus being SOLD to merchants when using the sell all button/key, The Necromancer skill spirit mark never registers the enemy its used on as dead which can break quests Some cutscenes crash repeatedly, Sometimes companions stop talking and become unresponsive in Skyhold, In multiplayer sometimes the key to progress doesn't spawn,

Sometimes in campaign your team gets stuck in combat mode even though all enemies are dead and are unable to level up or change equipment until you leave the sandbox area or load a savegame, NPC's sometimes sink into the ground, on previous gen consoles some people experience all savefiles vanishing Sometimes during conversations NPC's become completely unresponsive for a long period of time as if paused,

Sometimes the character record locks up making it impossible to level up and more
