UPDATE#2: The NOR gates no longer function as they did when I made this. A change that will keep it simple but effective is this: "good" switches connect to AND#1 gate. "Bad" switches connect to XOR gate. Connect a powered line to the XOR gate. Connect AND#1 and XOR to AND#2. Connect AND#2 to powered door or pressure plate based on your design.

UPDATE: Thanks to suggestions from Nacho Bidness, I've modified this design in two ways - 1) The inside has a pressure plate connected directly to the door to allow exit and 2) connected the output AND gate to another pressure plate outside that is then connected to the door. This means the combination must be entered then step on the plate but as soon as you step off, the door closes. Makes for an automatic door open/closer from both sides. Still requires computer to reset switches and number order still not relevant. See Nacho Bidness for a second version that includes those options.

In which I demonstrate how to make a combination lock for a powered door. Key things to know: 1) Pre-plan as much as you can, especially power. 2) Correct numbers go to a AND circuit box, incorrect numbers go to a NOR box. 3) Watch the wires to make sure they don't get in the way of wall placement.
