After giving up on this mission 20 years ago, I've finally finished Level 12 of the GTA III Ambulance torture. I thought I'd share the last level of gameplay with you for my "finest moments" playlist.

My plan was to narrate over this and give you some tips, but I thought perhaps I'll put said tips into the description and/or a blog post for the curious. Here's what worked for me, after tons of trial and error:

I did the mission in Staunten Island due to the wide straight roads and the ample time we're given per level. Previous attempts in Portland (too narrow, too little time) and Shoreside Vale did not work out well (the latter has a convoluted road layout and patients spawn in places too far to drive, the former has tight narrow roads and doesn't give us enough time).

The biggest issue I faced was when to do the mission: right at the beginning, Portland is peaceful, but towards the end we have two rival gangs chasing us down. In Staunten Island, it appears the Colombians keep attacking us, putting our ambulance at risk of destruction before we even get half-way through the mission. The solution: WAIT until Donald Love asks us to kill Kenji Kasen. This will instigate a gang war between the Yakuza and the Colombians, resulting in both gangs killing each other while leaving us alone. After this mission we can drive by the construction site without being shot at. I did this mission before the end of the game, and it worked beautifully.

The trick is efficiency when picking up patients. The time resets per level, so accumulated time does not roll over form one level to another, only within the same level. I tend to pick up the closest patients first, minimising driving time, and adding as much time to my counter as fast as possible. I've read a tip that we're supposed to pick up the furthest patients first, but I fare better when picking up the closest patients first, buying myself additional time early on in each level.

You cannot leave the ambulance, nor use a Pay'n'Spray to fix it. Drive as accurately as possible, drive safely and not too fast. Be aware of the erratic traffic and keep a distance if you can. Only 3 patients will fit into the vehicle at any time, so plan for a pickup round that's as efficient and safe as possible. I like using the staircase to drive down slowly, and the grassy patch to drive out of the hospital. I tend to use the sirens occasionally, but be mindful that traffic doesn't always move out of your way as you'd expect.

Got get'em, cowboy! After 12 levels, and 76 patients, you'll get a TON of cash and that sweet Infinite Stamina reward. Godspeed, Doctor!
