Timestamps and notes:

0:05 - Sierra 117 - Blind Skull (Normal+): Near the beginning of the level where you first encounter the covenant. From the waterfall, instead of continuing left, go towards the cliff wall on the right and onto a large rock overlooking the water.
0:28 - Sierra 117 - Iron Skull (Normal+): At the end of the level. As you cross the dam, jump onto a raised platform on the right and then onto a ledge in front of you. Follow the ledge all the way around and you will find the skull in a small alcove.
1:07 - Crow's Nest - Black Eye Skull (Normal+): At the start of the mission. Go to the back of the room, jump onto the shelves, onto a railing and then onto a pipe. Follow the pipe to the end.
1:27 - Crow's Nest - Grunt Birthday Party Skull (Normal+): After dropping down into a large pipe. Continue to the edge and drop down onto a small ledge below.
1:47 - Tsavo Highway - Tough Luck Skull (Normal+): After disabling a blue energy shield blocking the highway you will pass through a short tunnel. From the tunnel exit, go left and use a ladder to climb onto the pipe. Drop down from the pipe onto a small ledge and then onto the first section of wall below. Then jump onto a small rock platform containing the skull.
2:24 - The Storm - Catch Skull (Normal+): On a silo at the far end of the first open area of the level. You will need to grenade jump or jump from a vehicle to get onto the silo. Note: the skull will despawn if you destroy the AA wraith in this area.
2:48 - Flood Gate - Fog Skull (Normal+): At the start of the level when you first reach the buildings. Kill the flood as he jumps from the roof of the building on the right and it will drop the skull.
3:18 - The Ark - Terminal 1: After defeating the Hunter pair continue up the ramp along the cliff wall and into the cave. Enter the building and take an immediate left. To get the terminal achievements you will need to wait for the error and the new text to appear after it is resolved.
4:27 - The Ark - Famine Skull (Normal+): Once you are outside, continue onwards until you reach two ghosts under a large platform. Use the nearby rocks to jump onto the far end of the platform, then head towards the opposite end. At the final beam, grenade jump into the triangular area.
5:14 - The Ark - Terminal 2: In the building after the first section with the tanks. The second terminal is directly behind the panel used to activate the bridge. You will need to activate the bridge first to enable access to the terminal.
5:35 - The Ark - Gravity Lift: In the central bunker where you defeat the scarab. Required for the Cowbell skull further in the level.
5:49 - The Ark - Terminal 3: After defeating the scarab, head inside and pass through the room with the sleeping grunts and two hallways. You will come to a room with a ramp to your immediate left. The terminal is through a door that is under you, behind the ramp.
6:07 - The Ark - Cowbell Skull (Normal+): Exit the room containing terminal 3 and move to the next door leading to a downwards ramp. Deploy the gravity lift you picked up earlier near the end of the first ramp and use it to jump to the uppermost alcove behind the vertical energy beam.
6:26 - The Covenant - Terminal 4: In the first tower. Instead of going up the elevator, jump from the same platform to one opposite from where you came.
7:14 - The Covenant - Thunderstorm Skull (Normal+): After the the first tower, use the hornet to get to the second tower that was previously cleared by the elites. The skull is located on the outside of the tower on the central raised platform.
7:54 - The Covenant - Terminal 5: At the same tower as the Thunderstorm skull. The terminal is opposite the door to the building.
8:15 - The Covenant - Terminal 6: In the third tower, same placement as terminal 4 was in the first tower.
9:03 - The Covenant - IWHBYD Skull (Normal+): After dealing with Truth, jump through the rings in the order 4, 6, 5, 4, 5, 3, 4 with the first ring being closest to the elevator and the seventh closest to Truth. The skull will spawn at the lightbridge.
11:06 - Cortana - Tilt Skull (Normal+): About a third of the way through the level you will drop down a hole and into a circular room that still resembles a covenant ship. Use the flood structures on the left side of the room as platforms to jump to an elevated section in the middle of the room.
11:42 - Cortana - Terminal 7: In the same room as the Tilt skull, underneath the central area.
11:56 - Halo - Mythic Skull (Normal+): At the start of the level. Shortly after entering the tunnel, there will be a crevice in the righthand side of the wall. Follow this path to the end for the skull.
12:21 - Halo - Terminal 8: Shortly after the Mythic skull. Continue through the tunnel until it opens up and instead of dropping down to the Halo control room facility, continue to the right and there will a small opening to a building. Take a left and the terminal will be infront of you.
