Hideous Destructor | Midwich Elementary | No Failure Run | Part 2 FINALE
I used to play this map in V3603. The red skull key had a radius that apparently gets blocked by this map's door when played with HD, causing the locker door containing the skull to get stuck and me not being able to progress through.
Thankfully, Matt changed that in the latest version. It was about time I upgraded anyway.
