UPDATE: Apologies, it was 5am so I didn't know everything yet - will try to keep corrections here:
1 - I missed a quick step! After you have the access card you can use it to interact with a locker in Mutant Research (near the tank holding Richtofen's family) to grab a DNA vial you will need to activate the portal.
2 - When you get to the head step - skip ahead and do the orb step first, I think it's easier to do the orb before you do the head, it might be essential
3 - There are multiple leg spawns - we just got lucky and had them both in the same place in two games. They will be in robotic assembly somewhere, look for cabinets with the leg sticking out
4 - For the four-digit code - So each folder has a date and a note number - you can ignore the rest. Note down each date next to the note number, then put them in date order from earliest to latest (you only need to pay attention to the months) - now the note numbers should read the code.
So if you had:
03-27-1985 Note 1
11-22-1985 Note 3
01-13-1985 Note 4
06-07-1985 Note 6
You would put them in date order using the months (the first two digits) so:
01-13-1985 Note 4
03-27-1985 Note 1
06-07-1985 Note 6
11-22-1985 Note 3
So the code would be 4163
5 - Some people are having trouble getting the sprinklers working. For us they seemed to keep switching on and off - but try going through the metal detector, or throwing fire items like molotovs
