When you sign into a PS3 or Vita via the PlayStation Network, it will enforce two-factor authentication, causing you to have to scan a QR code, then get a password from the phone to use with the log in. Sometimes it does not work properly, even if you execute it perfectly (no miskeys.). The problem I, and others, have run into, is that the new password does not work, and the interface responds by giving you another QR codes. You end up repetitively going through the process over and over again. Basically you are stuck. Luckily, in my case, I was able to get it resolved. I believe it had to do with having multiple PSN accounts.
