I knew this moment is coming since I played the game earlier, but didn't realize it will get me crying again.
Found below from a random post on Reddit, but it makes sense.

Priscilla's song (Wolven Storm) becomes even sadder if you look at it from Geralt and Yennefer's point of view after a certain incident. Spoilers for the Lady of the Lake, the last book in the series. Proceed with caution.

Correct me if I'm wrong, but there's only one point in the entire series where Yennefer openly cries. That's when she's trying to bring back Geralt. Keeping that in mind;

The First Verse,
These scars long have yearned for your tender caress
To bind our fortunes, damn what the stars own
Rend my heart open, then your love profess
A winding, weaving fate to which we both atone

Geralt lying in pain wishes for just one more encounter with our raven sorceress hence the first line. He curses their destiny and how when they do come together somehow something tragic always strikes. But he still wishes to prove his love for her despite the believes about Witchers being emotionless. Their road to be together has always been full of hardships.

The Chorus,
You flee my dream come the morning
Your scent - berries tart, lilac sweet
To dream of raven locks entwisted, stormy
Of violet eyes, glistening as you weep

Yennefer's description and how one of them has to always part soon after they do come together. Geralt looks in her eyes as the last image of her in his mind is of her weeping.

The Second Verse,
The wolf I will follow into the storm
To find your heart, it's passion displaced
By ire ever growing, hardening into stone
Amidst the cold to hold you in a heated embrace

Here the view point shifts from Geralt to Yennefer, as she thinks to herself about how she'd follow Geralt through any storm, in this scenario, death. She'd willingly die with him if he dies and she's unable to save him. The second line represents how Geralt stops seeing meaning in taking sides and helping people for a bit during the end of the book, it's passion turning to more selfish reasons. She wishes to hold him and feel his warmth radiating, while in reality, he lies lifeless still and cold in her arms while she does everything she can to help.

The Second Chorus,

I feel like here's where the viewpoint shifts again, but not to Geralt dying thinking about her in the present, but how the next day, he'd be dead and she'll just be a dream. The last image of her, holding him in her hands weeping.

The Final Verse,
I know not if fate would have us live as one
Or if by love's blind chance we've been bound
The wish I whispered, when it all began
Did it forge a love you might never have found?

He remembers the last wish he made, how he chose to bind them together after he fell for her the first time he met her. His dying thoughts wander thinking about what would have happened if he hadn't done so. Would she still be caressing him? Would she even pay him heed? Would she have loved him? (This part especially guts me.)

The Final Chorus,

Here Geralt fades away, dying, he only remembers Yennefer's memory, her smell, and the pain him dying brings to her. How this is what makes her cry.

Keeping in mind how Dandelion was present there, through everything, I believed he's the one who wrote the lyrics encompassing both what his best friend and his best friends lover feel for each other. These he passed on to Priscilla and it became the Wolven Storm, the wolf indicating Geralt and the Storm his death.
