I'm (BCTE) putting in a partial description on behalf of Vtd. I don't remember all the details surrounding this one, but it's part of the little-known "Little Sister" series to Touhou, typically called Seihou or "Seihou Project". There were only about three primary games in its franchise. This game was, like most main Touhou titles, also of the danmaku shooting variety and it has various ZUN features, such as the music being composed by Zun I think.

Visually, aurally and generally, for a little-known game made back around 2001, it's pretty impressive when put side-by-side with the parent franchise, Touhou, and even brings about a different level of intensity. Kioh Gyoku (きおうぎょく or Ki-Oh-Gyoku / 稀翁玉) is the second game in the Seihou franchise I believe and it's a competitive versus shooter/scroller in the vein of Kanossyu, Dolucky no Puzzle Tour, Twinkle Star Sprites, etc. This game features females as well as a few male characters and a hidden Touhou character I believe.
