Getting all 8 fragments of reality in one run will ensure you get this ending and trophies. You can get these by either choosing the right door in the endless hallway (by either entering a breach or continuing to open a door after the room shakes) or finding the girl in the forest. Although I don't do this in the video, I recommend getting them before the monster appears on the 5th night as the second half of the game has a sanity timer.
