Turns out, you can just play the Amiga version on the DOS version given the right sound driver. I'm not 100% sure this sounds like the Amiga version, however it offers an opportunity to hear the environmental audio that is not present in the DOS version. There are also a few changes in the game code, but it's very trivial, so I doubt any bugs were fixed.
While the sound driver does initialize the MT32 MPU, based on how the game sounds, I'm going to say that the Amiga version doesn't actually use it and has the MT32 muted.
Note this play is done in one sitting, so any screwups and wandering around was left in, some on purpose. As some of the bodies can end up in different places between plays, it's a bit hard to plan an ideal play without leaving out something. Also some text parser nonsense happens from time to time.

Game played on DOSBOX 0.74-3.r4392-SDL2.0.12-MT32-FluidSynth
DOS resource files swapped with Amiga resource files, and using sound driver from KQ4 Ultimate patch. To play it properly ensure that only the resource.000, resource.001, resource.002, resource.003, resource.004, resource.005, resource.map, bank.001 and sound.128 files are copied from the Amiga version and replace the GOG version's. If your GOG version contains the text.052 and text.052 patches, remove those to avoid a bug late in the Amiga version of the game.
Alternatively if you wish to just play the Amiga version you can play it on ScummVM.
