Mercer is found, and Heather tries to talk sense into him. Failing utterly, Mercer sees his plan through to the end, namely to engineer a GUILT that could awaken his comatose wife, and Heather's mother, Tracy. Being GUILT, it has no intention of doing that and, after being awakened from it's beauty nap, goes on a rampage. Only one thing we can do to save Tracy life, what little life remains, and that is to operate on a person that spend years in darkness.

Operation time; weakened Bythos. "Weakened" referring to the patient, not Bythos. Since Tracy spend years in a coma her body simply doesn't have the strength to cope with being operated on, reflected by her max vitals. Bythos is still a slow bastard and, if your fast enough, easily dealt with, but this one took a level in badass. When it cuts the patient to bits vitals drop like a rock, and Tracy only has precious few to spare. The treatment plan is exactly the same as in Secret Patient, except that you have more vital damage and fewer vitals to work with. Again one of the hardest operations in the normal game, but since this IS the last chapter, minus Chapter X, that is to be expected.
