Mary Skelter Finale: Riley story end
2after the encounter with Guillotine and running back the way they came Riley is forced to make a decision which of her sisters will be allowed to survive. After making her decision both Riley and Erie are severely wounded and both fall off of a cliff. Afterwards we flash forward roughly 10 years later (I think) to realize that was Riley reliving her worst nightmares. Blaming herself for her attempt to save Rachel ultimately ending in failure and effectively abandoning her to be stuck with Guillotine.
Before you trigger this final event you'll want to check your map of the first floor again. There will be an optional event where the sisters encounter the wandering merchant. He will ask for his pocket watch back and Rachel will give it to him. He'll give his thanks but if this game is running by similar rules as mary skelter nightmares this is likely an important step for the true ending. That being said the game does seem to let you replay parts of Riley's story so you may not be screwed permanently out of the true ending if you miss the event but regardless I'd say get it now just to be safe.
