Early in Mass Effect 3 you can encounter former yeoman Kelly Chambers in the Docks Holding Area on the Citadel. She will only appear if you invited her up to your cabin for dinner in Mass Effect 2 and if you saved her from the Collector base at the end of the game. During this first encounter, she will tell you how she’s been utilizing her skills as a counselor to help the refugees.

If you encourage her to continue helping the refugees then she will be tragically killed during the Cerberus coup. However, if you tell her to change her identity then she will change her name and hairstyle, which saves her life when Cerberus attacks. If you come back and speak with Kelly after your first conversation with her, then she will let you know that she was spying on you while working as your yeoman in Mass Effect 2.

Assuming you also asked her to change her identity, if you forgive Kelly then she will survive for the rest of the game… or at least until the Citadel is captured after Cronos Station.

However, if you do not forgive her then she commits suicide. You can overhear about her death from Ken and Gabby on the Normandy.
