My strategy for the torture sequence is to tilt my controller a bit with my left hand and hold it for support and use my right hands middle finger to mash the circle button. You want to make sure you are in comfortable position doing this because if you are uncomfortable you'll probably struggle a bit. I'm able to get through all 6 rounds on extreme mode doing it this way. If you are struggling to hold the controller than you could try laying your controller down and mashing the circle button with your middle finger. whatever you do just don't use your thumb. Your middle finger is actually stronger and faster for mashing the button if you are doing the button mashing correctly.
