To unlock Mission 46 "Truth: The Man Who Sold the World ", Phantom Pain's true final mission you need to meet certain requirements.

Side Op - Extract the AI Pod
Side Op - Secure the Remains of the Man on Fire
Side Op - Search for the Escaped Children 1
Side Op - Search for the Escaped Children 2
Side Op - Search for the Escaped Children 3
Side Op - Search for the Escaped Children 4
Side Op - Search for the Escaped Children 5
- Listen to all story audio/cassette recordings (they are yellow in color)
- Upgrade entire base, and platform to level 3 or higher.
I just completed missions 46, and didn't play any of the repeat missions 32-51.
Instead I just kept doing side-ops, and returning to Motherbase regularly to unlock the new story missions and side ops.
I have spent the last few hours waiting for my entire base to get to level 3 or above, and 46 hadn't been unlocked. So on a chance, I listened to all the story audio and bam, I noticed my missions were highlighted yellow meaning a new story mission had been unlocked.
So, I don't know if level 3 base is needed or not, but many seem to think it is.
Finally, the 2 Quiet side missions are completely optional and not needed for progression.
Side Op - Secure Quiet
Mission 45 A Quiet Exit
Mission 46 is treated as the true ending of MGSV. It's quite peculiar as you will be told to repeat the prologue (0-Awakening). You are basically do the same things as on the first time and any changes appear only in some of the cutscenes. Mission 46 however, ends earlier than the Awakening. You won't be escaping on the horse from Man on Fire.
Important Note! Don't forget to watch additional cutscene which plays after mission summary.
Tasks in the mission
OPTIONAL (1/2): Eliminate 6 soldiers at the hospital entrance) - Soldiers refer to the enemies in the hall, right before leaving the building. You can use the default pistol or some better gun you will pick up from an enemy.
OPTIONAL (2/2): Land 20 or more shots on the Man on Fire - Start attacking him right away as he appears in the hospital hall. Use the covers and take any additional ammunition from the enemies nearby. Leave the hospital as soon as you land 20 shots on target (as by no means you will be able to defeat him).
Important Note! On the task list the main task does not appear. This would be escaping the hospital.
Preparations
You can't prepare to this mission. You will begin the same you did in mission 0. Later on you will be given a pistol.
