It is a lot rarer in v1.1.1. Ran into it about 4 times out of 200+ games. About half the time, it can be fixed by spamming the secondary weapon button. Took a while for this occurrence but it eventually resolved. Could also have to do with getting a kill, or 1 cooldown after getting a kill.
