I wanted to share a vid of FINALLY getting an S-Rank in a level, which seems to be a very high bar to set. You can skip to the end to see my stats, but I will post them here:

Robots Killed: 95% (I think this is the biggest factor in your rank, more below)
Headshots: 144
Finishers: 7
Exploded: 26
Electrocutions: 1
Grinded: 3
Frozen: 0
Slashed: 0
Nutshots: 4

So, the first thing I noticed is that this is the first time I have killed 95% of the Robots, and I was wondering if this was why I kept getting A rank. I would think this is the biggest factor because I have pretty similar stats in all other fields, but always around 89-93% of Robots killed.

That being said, if you play on No Timer mode (I am right now for the achievement), you no longer have a 10 second life span, but a health bar like during the boss fights. Basically, you can wander around at your own pace, and if you go back to doors that you didn't go through when you reach splitting paths, you can kill more enemies instead of just skipping those rooms! So make sure to backtrack to doors you didn't go through! Also, don't use vents for shortcuts, as you also will miss enemies this way.

Since you have unlimited time in No Timer, take the time to line up headshots! I am sure this is also another big factor in the rank at the end, and since you have time, use it and make it count! I didn't go for many environmental kills, but it didn't seem to matter for the ranking. Use any Items you pick up like Knives and Manga Books since that is the third thing in the list (Finishers), but outside of that, I would just aim for the head.

Try not to take too much time looking around. I was searching around because I still haven't found the Joke Weapon in the game yet, but was trying to not take too much time. I finished over 10 minutes, so it doesn't seem like you need to be under 10 minutes to still get an S. I just wouldn't end floors at 2 minutes or more, the time it takes to beat a Chapter may be a factor.

If you are having trouble doing this, give these tips a try! It definitely feels like a tough achievement to get unless you set up to actually have the chance. In the Normal modes, it's too fast paced, and you can't really backtrack as easily. I am sure it can be done, but you might as well make it easier, and not as stressful.
