This is part of the Shadow of the Abyss DLC.
This video covers Tia unmasking, and getting the sword and beast charms. Here are the timestamps and notes.

0:00 This video begins with Kirito reuniting with his group after him and Tia fell off a cliff.
2:36 Tia takes off her mask
3:44 The group head back to town. Tia will now join your party automatically for certain story events, but you can't put her in the party. She just shows up on her own. After the scene you will be back in town. Make whatever preparations you need to make, and then teleport to Fiernia Sanctum.
4:44 In Fiernia Sanctum go to the alter in the south east.
5:14 With the Humanity Charm and Snow Charm you can now activate the alter. When asked by the alter, pick the Humanity Charm first then the Snow Charm second. This will unfreeze the teleporter nearby. Use it to warp to a new area.
Note: I hope you like platforming, because the rest of the DLC is full of platforming sections.
5:49 In this new area, called Illustia Air Gardens, you will have to dash across island to island to advance. Your goal is to head straight south. You will eventually find a alter on the way south, it will have a quest marker on it.
7:00 Interacting with this alter will give you a tip. Nothing else can be done here, so continue south. In the most south eastern island you will find a warp totem.
7:56 Use the warp totem to move on to the next area.
7:59 Sacrifia Sky Chapel. In this area head to the south. The middle of this area is split into north islands (grassy islands) and south islands (desert islands). In the middle of the south islands you will find the next alter.
8:31 Interacting with this alter will give you a second tip. Nothing else can be done here. Continue on to the south western most island.
9:46 Take this warp totem to the next area.
9:49 Straghorn Sand Wastes. Head south and then west. You should see a grassy rocky island.
10:30 Once you see this rocky grey area (its in the middle of the map) you have to climb the mountains to the west of it and dash over to the rocky island.
10:38 Once on the grassy area, stay on the islands and head south to another Altar (Altar of History)
10:45 In this Alter you have to fight some enemies. Tia will join you for this battle.
11:41 After the battle, Kirito will inspect the Alter to get the "Sword Charm". Teleport back to Sacrifia Sky Chapel.
Note: The following might or might not be optional. Going to Sacrifia Sky Chapel will only give you a hint on what to do next, but won't progress the story. This might be how the devs intended to point you to the right direction. I did this just in case it wasn't optional. You can skip to the Oltado Cloister (14:23) if you want to skip straight to the next thing to advance the story.
12:50 (optional?) In Sacrifia Sky Chapel head to the large island in the north west. In the very north of the large island you will find a giant teleporter.
13:29 (optional?) Inspecting the large teleporter will give you a scene saying it wont work. Interacting with the alter will let you pick charms. No matter what charms you use or in what order it wont work. Instead the alter tells you to go to Oltado Cloister. Teleport to the Oltado Cloister.
14:23 In Oltado Cloister you have to climb up to the high mountains in the north. You will find a giant rock (14:33) that you can run up and dash over to the mountains. Head to the west until you find an alter.
Note: I am sorry for the poor directions, but at this point it gets pretty hard to outline what to do.
14:55 Interacting with this alter will let you pick charms. The proper order is: Sword, Snow, Humanity. In this order the alter will spawn platforms. You need to use these platforms and head west.
Note: I cut out me just failing at this for 10 minutes. If you have double jump or glide this is easy. But I just cut to the time I did this successfully.
15:54 After using these platforms you will eventually reach an alter high in the mountains.
16:17 Using this alter will throw you into a boss fight. Tia will join the fight.
17:06 After the fight, the Alter will let you switch a charm for another. ONLY PICK THE BEAST CHARM. Don't pick any other one. Once you are ready teleport to Sacrifia Sky Chapel
18:29 Once again in Sacrifia Sky Chapel head to the large north west island
19:03 Once you arrive at the alter here, interact with the alter and pick the charms in the following order: Beast, Snow, Sword. This will unlock the teleporter. Use the teleporter to go to a new area.
19:48 Oldengard: Path of Purity. Once here try to head east. You will be thrown into a boss fight. Tia will join.
