I was playing vanilla Half-Life from Steam; not Source, and without any mods or file changes. I had just downloaded the game and verified my files.

This particular lift would insta-gib be every time there seemed to be any items or corpses on top of it, which is a common occurrence if you happen to mine-trap the lift exit. The damage was so insane that it would send the camera flying up to the room above. Destroying all corpses and picking up all the items on the lift seems to have solved the issue. I have no idea why having items/corpses on the lift causes this bug.
