I use the Gulliver Reborn mod to set my player size to 0.1 for an entire survival playthrough (it isn't finished yet, but I still stay small the whole time). The biggest difficulty with being this small is how long it takes to mine anything.

Gulliver Reborn is a remake of the gullivers mod. It lets you change your size and gives you abilities depending on your size. Some of these abilities include climbing dirt/sand/gravel while small or stomping mobs while big.
