Around halfway through the Doom II portion comes one of the trickier maps to pistol-start. The best way to increase the chances of survival early on is to grab the SSG guarded by the lone Cacodemon, and then shoot the two hitscan-triggers to unlock a Megasphere, Backpack and 40 shells.

After that, the route taken to clear out the infestation is entirely up to the player, although grabbing the BFG9000 and cells before taking the Blue Keycard is heavily advised; being swamped by the clusterfuck of hellspawn that teleports in with only minimal weaponry at your disposal is a sure-fire way of being killed within seconds, as even with all the infighting that can happen, there are far too many stray fireballs from the Mancubus-heavy ambush.

And yes, I do go for the secret exit.
