I go back and defeat the Nihils, takes a couple more tries. Here's the strategy. I recommend using both Ancient Magicks and Curses. Have the best magick armor you've got, the best beast of burden you've got with the best food, and a Staff of Light or better. I like the Staff of Light because of its ability to save runes. Use the best Ancient Blood magick you have.

Fight the Blood Nihil first, then either Smoke or Shadow, and finally Ice. Each of them uses a special attack, which is signaled when they "hiss."

Blood will likely be the hardest one, so kill it first. Just keep attacking, protect or deflect melee (to lower damage), and keep healing when you get low. If you see another Nihil charging, GET OUT OF THE WAY. They only go in a straight line and will deal damage if they charge passed you and some lower stats.

Smoke and Shadow will be fairly easy, just try to avoid attacking multiples at once. Smoke's special attack with drain your stats, avoid it and use a Super Restore potion if you get hit. Shadow's attack will deal heavy rapid damage, definitely move when it uses its special. For Smoke protect range and Shadow protect magic; don't take my word for it as I can't quite remember which is which! One is range and one is magic.

Ice will freeze and stun you. This one is annoying, and can kill you if you're not careful because you CANNOT heal when you're frozen. Make sure you keep your health up at all times. Protect/deflect magic against ice.

If you want to try and get the Anihilator title, you need to have all of them attacking you at least once during the battle. Despite popular belief, you do NOT need to kill them at the same time. For Annihilator, bring blood down to next-to-no health, then Shadow and then Smoke. Once these three are almost dead, kill Ice. Then kill Shadow and Smoke and finally Blood. The reason you should attack blood first and kill Blood last (if trying to get all of them attacking you at once) is because Blood will heal the others.
