This achievement requires you to not pick up healing items, or take blessings that heal you directly. The only means of healing I had at my disposal was one of the cursed (green) blessings called Cursed Vitality, that reduces your max HP to 4, but heals you for 1 every time you acquire/spend enough blood (the game's in-run currency).

It was really hard! :D

1% of players have this achievement, and I suspect the percentage is significantly higher than it 'should' be, after reading a couple of people mention that turning on invulnerability in the accessibility settings doesn't prevent you from earning the achievement. No cheese here, though! 🐭🎶
