It looks as if a Second Polybius Video has been found. This time the Cabinet seems to be from a Vertical Arcade. The video is a better resolution the then last video, however this one looks as if it has different game play.

The story goes that shortly after its release, players began reporting nausea, lack of sleep, headaches, night terrors, and other psychological symptoms which they believed were caused by playing it. Some players reported experiencing these symptoms even after only one play session.
The game was removed from circulation shortly after its release and never seen publicly again. There is no evidence that such a game was ever released at all.

The legend of Polybius is an urban legend concerning a fictitious 1981 arcade game. The legend describes the game as part of a government-run crowdsourced psychology experiment based in Portland, Oregon. Gameplay supposedly produced intense psychoactive and addictive effects in the player. These few publicly staged arcade machines were said to have been visited periodically by men in black for the purpose of data-mining the machines and analyzing these effects. Allegedly, all of these Polybius arcade machines disappeared from the arcade market.

Polybius is one of many legends surrounding video games that were supposedly too good for their own good. From the infamous "Pac-Man" death screen to Pokemon GO's reported link to mass suicides, there are countless stories about games that were so addicting they caused players—and even entire communities—to go crazy.
