This is the World D-4 Level from Super Mario Bros: The Lost Levels for the Nintendo Entertainment System.

This level starts off as a slightly modified version of World 8-4, but this time you're actually expected to make it to the end of the first area. Then you are taken to a windy outdoors area with some enemies. Next you take an underground path, and that takes you back inside the castle. There is the easiest fight with Bowser's Brother. You can literally just run underneath him. After some tricky jumps, it's the fight with Bowser, and then the game ends with the same ending as the regular game.

This level would have been nice if it wasn't for a few of those tricky jumps. It still isn't worth the effort to get to it.
