Es un juego del género 'beat em up', de esos que invadían las recreativas en los noventa, en donde avanzas por un nivel y vas golpeando a tus enemigos hasta acabar con todos.
Maximum Carnage es legendario por cartucho rojo y por seguir fielmente la historia del cómic que lleva el mismo nombre.
La historia comienza cuando Carnage escapa de prisión y con la ayuda de Shiek, Doppleganger, Demogoblin, entre otros villanos, inicia una masacre en Nueva York. Spider-Man, Venom, Captain America y Black Cat deciden detenerlo.
