01:03 - As Kaede, reach into the toilet and get Mugen's smartphone.

For some reason Mugen has one again when he enters the village -again- ?!?!

26:44 - As Mugen, use the camera (even if the game prompts the smartphone) and take a photo in the room full of eyes. Now you can open the closet and instead of the boy, you find a note and the toy dog that Otoha gave him.

And sit back, watch the weird ass ending.

So to sum things up. You gotta play through normal end to get true end - and you are going to play the end twice for that reason, with just this one thing changed that gives you a weapon or something.

Saki loses photo.
Shinji picks up photo & leaves crowbar. (gives true end)
Kato hunts down Otoha and nerds.
Otoha leaves toy dog to ghost boy & picks up crowbar. (gives true end)
Kato takes photo from Shinji's ghost hand. (gives true end)
Saki calls professor which is prompted by Kato picking up the photo, killing the professor. (gives true end)
Mugen loses smartphone in toilet.
Kaede picks up smartphone in toilet (gives true end)
Mugen picks up toy dog left by Otoha, (gives true end)
