My first perfect Lunatic Touhou clear! As of writing, the second hardest thing I've done yet, just behind Celeste's Farewell Golden.

0:00 - Stage 1
1:50 - Stage 1 Boss Eika
2:40 - Stage 2
4:50 - Stage 2 Boss Urumi
6:04 - Stage 3
8:55 - Stage 3 Boss Kutaka
10:24 - Stage 4
13:30 - Stage 4 Boss Yachie
15:00 - Stage 5
17:34 - Stage 5 Boss Mayumi
19:05 - Stage 6
20:45 - Stage 6 Final Boss Keiki

This Touhou game is pretty different compared to most. With the exception of the final boss, the spellcards are all very easy and/or static, with the nonspells posing a greater threat and having more RNG. The stages also have significantly more streaming than usual, the biggest offender being the incredibly boring Stage 3 until the very end. The biggest threats in the game are the precise attacks of the final boss Keiki, Mayumi's nonspells, and to a lesser degree, Yachie's nonspells.

Thus I would recommend this game for people going for a perfect Lunatic Touhou 1cc but have more experience in games like precision platformers and less experience with reading bullets in bullet hells proper. Its also one of the easiest Touhou games to get a perfect Lunatic run on, in large part thanks to Youmu. Many dangerous attacks are completely neutralized by Youmu's outstanding damage output, whether against a single target or multiple.

Usually it is said that the final boss, Keiki, represents most of the difficulty for a LNNNN. In my case though, I found Mayumi's nonspells to be biggest obstacle. Still, I would agree in general that Keiki is the hardest, though, since by the end I also got pretty consistent at Mayumi after practice and refining strategies, while a majority of my total hours consisted of practice solely for Keiki. Total attempts on Keiki spellcards sum to around 4000, while total attempts on non-Keiki spellcards sum to around 1500. This run was my second run ever that made it past Mayumi into Stage 6.

Overall this took ~70 hours playtime in WBaWC, out of ~230 hours in Touhou and ~250 hours in bullet hell games in general. As usual for challenges of this difficulty, most of the time was spent practicing attacks rather than recklessly attempting runs. I found it a bit easier than Celeste's Farewell Golden, but I'm hesitant to objectively compare their difficulties. I have double or even triple the amount of hours in platformers, but I also think I improved greatly in general as a player since Farewell Golden, and many skills are transferrable. In the end they're probably around the same level and I expect people will probably find one harder than the other based off their own strengths and weaknesses.
