Link can use the Ultrahand to scale the cave's farthest left wall, reach the shore, and retrieve a nearby wooden chest that is slithering in the water. Link may then claim the chest, which contains a Sapphire, by attaching it to the wooden board he passed in the river and dragging the whole thing over to him.
