For anyone else who wants to leave the twitch affiliate program here are the steps
1. Make a twitch support ticket
2. There should be a combination of category and sub-category. The sub category you need is "offboarding" (I think. I don't have the option anymore because I'm not an affiliate. If you find it, please let me know)
3. Provide this information TwitchID/Username, IP (link.twitch.tv/whatsmyip), birthday, Transaction ID of the last purchase made on twitch (You can find this on your bank''s statement), email address.


The information in point 3 is what I was asked for. You might have to provide different information. But, It will make the process faster if you provide info from section 3.
