Step 1 - repair your ship
Step 2- acquire the hyper drive
Step 3- acquire your first anti matter to craft a warp cell. (go to your hyper drive and put the warp cell in it)
Step 4- go to a different star system meaning (on ps4) hit down on d pad, select a different star system and travel there
Step 5- In the second star system use L3 to perform a scan
Step 6- there should be an operations centre or something that pops up
Step 7- go there and there should be a system, interact with it (I don't remember if it gives it to you automatically or you have to do the 3 choice selection like every ops centre, if you do then you must get it correctly)

Note It is possible for some peoples game to not be the same, it is procedurally generated but for the most part people have confirmed this works. If it doesn't I would hit the reddit forums to get more information. I put this video out there as fast as I could when I figured this out, some people have gotten it by recreating these steps some have not, please realize that is out of my control and nothing wrong on my end!

EDIT: YOU CAN SCAN MORE THAN ONCE AND FIND MORE BUILDINGS AND SUCH!!!!!
