Blank game phenomenon - Getting a disc/cartridge with nothing on it

I have heard of this at least five different times in my life. Someone buying a game, but the disc, or cartridge being blank. I believe this is possible, but as to the validity of these claims I have read, I am not sure. Have you ever heard of this happening?
