In Darkest Dungeon 2, the brain of darkness, with this current patch (29Nov2021) is really hard. Between the four shackles, there are good stuns damage, and hp. I would focus the latch of regret, despair, fortitude, and then lamentation. Obviously, your playstyle and party will dictate otherwise. Bring plenty of blinds and healing items as your first 5 turns will be the roughest as 4 shackles pound into you.
