BRUTAL DOOM Insanity Nightmares,a fork of Brutal Doom v20 that adds a lot of new monsters together with arsenal of new weapons and even items, made by killerfrosty plus Project Remap, No Pain No Gain difficulty, 100% Secrets achieved. Brutal Doom Insanity Nightmares surely has strong vibes of Insanity's Brutality (a mod last updated in 2017) while playing, even some sound effects are the same, climax and gameplay are also quite close to it. Remember that Insanity's Nightmares is a Brutal Doom v20 fork so you might encounter some performance issues such as spikes or general drops, it is advised to turn off some options or decrease resolution to avoid such issues. The No Pain No Gain difficulty is an obvious reference to the Shadow Warrior and is an Ultra-Violence counterpart from there, but because of the various bonuses dropped from the enemies it might be a little bit easier. Modification also has a quite nice inventory items added to the spawns, for example you can pick up a Medicine backpack that can restore your health up to 100 if you are injured, a nice thing when you are in serious trouble. But the real healthsavers will be your turrets that can be found mostly inside these wooden crates - there are two versions of them that you can find, a light machine gun version and a minigun, both of course have different visuals and stats. To use them you must simply come to a place where you want to put them down and press a button responsible for item's usage, the turret is ready to fire at your enemies but keep in mind that these can be destroyed by demons when engaged. You can place these turrets whenever you want, when the area is clear just come next to them and pick up them back, they are truly a great item that will help you a lot while playing, lots of encounters are way easier thanks to them. The gunplay is something that you need to get over with while playing, the recoil and spread is somewhat large while using some weapons, while such bigger guns like Nailgun or Minigun are way easier to control in my opinion. As for Project Remap it is a WAD that 4 DOOM 2 remade and enlarged a lot, addins lots of new areas and rooms for you to explore, together with removing some very easy secrets or adding new secrets that are more difficult to find. There are of course way more enemies for you to eliminate, hundreds of them are waiting for you on each map, lots of them will be teleported nearby you, the WAD author surely loved to place tons of Pinkies to surround you.
