This ending is obtained in the "New Game Plus" mode by beating Lavos after you get the Masamune fixed and get frog in your party but before you fight Magus for the first time.

Once Lavos is beaten and a brief intermission occurs, frog will go off on his own to take care of some unfinished business. The scenes that follow show him making his way through the castle confronting each of Magus's henchmen. While the credits roll sounds from the epic battle can be heard as Frog engages Magus. At the end a cloaked silhouette, which looks like frog, is standing on top of Magus's castle.
