I finally pulled off full game hitless on Nightmare, after nearly 40 hours trying for just this after getting all achievements, and 76 hours in game. Sniper seems like the way to go, you can pretty consistently get the damage to kill bosses/threatening enemies in a reasonable amount of time, and use grenades/melee to help with the trash mobs. Alternatively, I was also trying for Minigun runs and just hoping for flat damage upgrades to make it insanely powerful, but its not as good for quick turns to shoot then spinning around again to keep strafing/dodging away from enemies at full speed.

For hitless, the main concerns are the 6th+7th islands of each area, where you have to deal with multiple tough enemies at once. In particular, the 2nd area (Arctic Biome) is really tough, a number of maps are really easy to get trapped on or just fall off the map. Sometimes, you get lucky on the 2nd elite enemy and they just jump off the map like they did in this run, but its really inconsistent and you can't plan for it. 3rd area isn't really that bad, but the pressure of the run getting there is enough to force some chokes, and then the last boss is usually incredibly tanky so you'd better be great at dodging or pack some serious damage. I chose the latter. Note that while parkour islands are excellent for no hit, its impossible to take damage, they won't count as a "flawless" island either so you'll end up with a lower results screen flawless count -- this gave me a scare for a moment when someone pointed out I only had 23 flawless islands.

Crab Champions is a really fun roguelike from Noisestorm, it's fairly punishing on Nightmare difficulty, even trying to beat it normally and not worrying about hits. The game is Early Access, but is very playable and fun already. The game is on Steam here, and I highly recommend:
