This is Doom 64 Retribution Level 31: In The Void - done with all kills and secrets on Watch Me Die-difficulty.

At last we get the third Demon Artifact, which upgrades the Unmaker to its most powerful form. It fires 3 laser beams, making short work of even the deadliest enemies of the game.

In The Void is the thirty-first level of Doom 64, and it is also the last secret level of the game. This map resembles Duke Nukem 3d's space station maps a bit, and you can see clever use of fake 3d geometry here.

To get the last Demon Artifact, you must almost complete the level to trigger an earthquake, press a switch and go to the room where the Blue Key was. The Demon Artifact seems to have vanished as you press the switch near it (near the Red Key). When it vanishes, go to the Blue Key area, and grab it at the far end of the room where you have a small raised platform leading to that weird wooden structure.
